16:40 exact the opposite: patients with degenerative disc disease resulting in stenosis are comfortable in sitting as opposed to walking and standing, anatomically the spinal canal being narrowest in lordosis. Old stenotic back patients tend to deviate in more or less flexion.
14:55 Pain in sciatica is caused by disc praesure (protrusion) on the segmental nerve sleeve resulting in dermatomical segmental pain from L4 to S2. Nerve ischaemy caused by disc praesure (herniation) results in numbness and motor loss. So just pain is an indication for manipulation/traction, nerve signs are contraindications.
Why does this first speaker ignore the evidence based Orthopaedic Medicine (Cyriax) diagnostics and treatment arsenal? Muscle spasm as is usual in acute backpain in people up to 50 in age is caused by disc instability or even disc protrusions with signs of asymmetrical loss of back mobility, pain when coughing, positive SLR, mostly unilateral muscle spasm. Never met a 55+ aged patient allegedly suffering from acute spasm from facet joint athritis.

ReplyDelete12:20 What about surgery for bladder disfunction caused by large central disc herniation on S3,4 at L5 level? Is this a lecture by an expert?
